Office visits are scheduled by our receptionists. Scheduling of surgery is done by the office coordinator.

 

Except in the case of emergencies, it is not our policy to see patients without an appointment. Our schedule is often filled well in advance, and we do everything we can to assist you if delays or postponements occur.  At times, our doctors are called to the hospital, or some patients require extra clinic time for their serious injuries. We appreciate your understanding and patience with us as we do the best we can in managing our time constraints.

 

If your time is short, we encourage you to call ahead and check our clinic schedule time. We will be glad to accommodate you at a later time that day.

What to Bring to Your Appointment

Please Bring:

  • Your insurance card and a photo ID. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with your carrier if you are in doubt.

  • Athletic shorts (hip and knee examinations only.)

  • Clinic notes and operative reports from any previous surgeon or physician you have seen relating to your injury.

  • X-rays or MRI's not older than 12 months old.

Workman's Compensation

For patients whose injury is work-related, you must provide the clinic with an authorization letter from your employer or agent. Please include a claim or case number, contact agent and their phone number, and appropriate billing address.

Rehabilitation and Recovery

If your injury requires surgery or physical therapy, we understand the importance of rehabilitation to your treatment. Often customized rehabilitation programs are designed to meet your specific need. 

Post-Operative Instructions

These post-operative instructions are general guidelines to aid in your recovery and do not apply to all patients. Please follow your specific post-operative instructions and contact our office with questions.
